How does your clinic operate, i.e. services rendered, billing, staffing, hours, space, self-managed or managed by another group, etc?

Our "clinic" is called Independent Living Support and it runs 24 hours staffed by CNA/MedTech's and 2 RN's (during the day) to meet the needs of light home health items of our Independent Residents. This is an included service under our contract so no additional billing. It is fully staffed by our own employees, no other group.

At Arbor Acres our clinic is staffed with 2 nurses and our Medical Director sees IL residents 2 days a week and then has a PA in the clinic 5 days a week but only 3 of which are all day. Both of his days are 1/2 days.
Our hours are 8:30-11:30 and 1:00- 4:00 Monday -Friday and 8:30-12:30 on Saturdays.
When we have both nurses working in the clinic they one of them responds to the emergency calls in IL as well.
The Medical Director has his own private practice and all billing is done through his business office.
We have 2 office spaces and 2 exam rooms.

Our Clinic operates like a PCP office. We are open Monday-Friday 8:30-4:30pm. We are staffed with 3 Geriatric physicians, they are not here on the same days. We have a Nurse Practitioner that sees patients 3 days a week. Twice per month we have a podiatrist and a psychologist in for Clinic visits. One half day per week we have a Geriatric Pharmacist in the Clinic. There is a nurse on staff during all Clinic hours that not only assists the providers but also does nurse visits such as: scheduled dressing changes, injections, BP checks, etc.

In order for Members to see providers in the Clinic, they must have an initial visit such as with any PCP office and establish care. The only exception is podiatry and psychiatry. Any Member can be assisted by the nurse for nurse visits without joining the Clinic but they have to have orders from their own Physician/Provider. **I attached a list of our Providers schedules for further details.

At Springmoor we maintain clinic hours 7 days per week, 8:30 am to 5:30 pm. M-F most days are staffed with a nurse practitioner and a physician, which are contracted through a local medical practice. We also have two nurses and a secretary M-F and a nurse and a secretary Sat and Sun. The nurses are Springmoor employees. We offer, Audiology, Mental Health Services, Dentist, Dermatologist. Doctor visits are billed by the provider. Items supplied to residents by Springmoor are charged on their monthly statement.
