Here is a list of her job responsibilities:
Assessments of IL resident’s needs
Cognitive evaluations
Education and recommendation of the services provided to our IL residents
Assistance with Home Care admissions
Assistance in transitions to higher levels of care when needed
Dementia education for residents, families, and staff
Care Coordination services to residents who contract for additional help in arranging medical appointments, transportation, etc.
Works with doctors and sometimes lawyers on issues of competency with residents
Counseling on a wide range of mental illnesses, caregiver stress and fatigue, etc.
Coordinates the Crisis Intervention plan and team; Trains frontline staff on the process of crisis intervention
Helps to facilitate the Dementia Support Group and the County Parkinson’s Support Group
Staff training and education
Participates on committees: Outreach Committee, Ethics Committee, End-of-Life Committee

They are there to assist with questions about transitions between care levels, hospital discharge processes, long term care insurance, and other inquiries that come up about living in our community and the healthcare system in general. They also lead and/or assist with support groups and resident education (Advanced Directives, End of Life, etc.).
