Advance Care Planning Resources
Share |

Advance care planning helps your loved ones and health care providers honor your goals and wishes if you are unable to speak for yourself. If you are ever in an accident, have surgery, or get a serious illness you may need someone to advocate for your wishes. Consider reflecting and writing down your wishes with help from the NC Partnership for Compassionate Care.

LeadingAge NC is pleased to be a member of the statewide steering committee of the North Carolina Partnership for Compassionate Care. The mission of the Partnership is to "ensure that patients’ end-of-life care choices are openly discussed, documented, and honored. Our aim is to provide educational resources to the community and health care professionals to improve the quality of care at the end of life."

Visit http://www.gotplans123.org to learn more about advance care planning and check out NCPCC's website for a wide range of resources for professionals and lay people including advance care planning forms, educational videos, and more. Get the conversation started!
